Once your appointment is confirmed, please plan to arrive 15 minutes early and bring the following items with you:

• Photo ID
• Current Insurance Card
• A list of all prescriptions you are currently taking
• A valid means of payment (i.e. debit or credit)

Copays, deductibles, and outstanding balances will be collected prior to your appointment.
