Central retinal vein occlusion (CRVO) involves the main retinal vein where all branches meet and typically causes more widespread damage. BRVO is four to six times more common than CRVO. Because BRVO affects a smaller area, it may cause less extensive harm, though vision loss can still be significant when the macula is involved.
One serious complication of BRVO is retinal neovascularization. When a portion of the retina receives too little blood, a condition called ischemia, the body may respond by growing fragile, abnormal new blood vessels on the retinal surface. These vessels are prone to bleeding and scar tissue formation, which can lead to further vision loss or even retinal detachment if not treated promptly.
Risk Factors and Who Is Most Affected

Uncontrolled high blood pressure is the single most important systemic risk factor for BRVO. Elevated pressure damages and stiffens artery walls throughout the body, including in the eye, making it more likely that an artery will compress a nearby vein at a crossing point. People with a history of stroke or cardiovascular disease face a similarly elevated risk because these conditions share the same underlying process of blood vessel damage.
Diabetes mellitus contributes to BRVO risk by damaging blood vessel walls and promoting abnormal clotting. When diabetes is combined with high blood pressure and high cholesterol (a cluster of conditions known as metabolic syndrome), the risk for retinal vein occlusion increases substantially. People who have experienced organ damage related to both diabetes and hypertension face a particularly elevated risk.

Recognizing the Symptoms
BRVO often announces itself through sudden, painless changes in vision. Knowing what to watch for can help you seek care before the condition causes lasting harm.
The most recognizable symptom of BRVO is a sudden, painless change in part of your visual field. Many people notice blurred or distorted vision that may appear all at once or worsen over several hours or days. Depending on where the blockage occurs, the area of blurring may be small and off to one side, or it may affect much of the vision in the eye.
Some people with BRVO notice floaters (dark spots, lines, or squiggly shapes that drift across vision). Floaters related to BRVO are caused by small amounts of blood leaking from damaged retinal vessels into the vitreous, the clear gel that fills the inside of the eye. If the blocked vein is located away from the macula, BRVO can sometimes go unnoticed because central vision is not immediately affected.
Any sudden, unexplained change in vision deserves prompt medical attention. Treatment Options for BRVO

The primary treatment for macular edema caused by BRVO is a series of intravitreal injections (injections given directly into the eye) of anti-VEGF medication. VEGF stands for vascular endothelial growth factor, a protein that promotes abnormal blood vessel growth and fluid leakage. By blocking VEGF, these medications help reduce swelling in the macula and can meaningfully improve vision. Several anti-VEGF medications are used for BRVO, including aflibercept (Eylea), bevacizumab (Avastin), ranibizumab (Lucentis), and faricimab (Vabysmo), a newer bispecific antibody that may allow for longer intervals between injections. Because macular swelling tends to return as the medication wears off, anti-VEGF injections are typically given on a regular schedule. The retina specialist monitors your response using OCT scans and adjusts the treatment interval based on how your eye is doing. Focal laser treatment is sometimes used for specific cases of BRVO. When BRVO leads to ischemia and the growth of abnormal new blood vessels, a different type of laser procedure called scatter laser photocoagulation may be recommended to reduce the risk of serious complications such as bleeding inside the eye. Research has shown that scatter photocoagulation significantly reduces neovascularization-related complications.
