
Central Serous Retinopathy: Causes, Symptoms, and Treatment
What Happens Inside the Eye
To understand CSR, it helps to know how the layers of the eye work together. The retina relies on several supporting structures to stay healthy, and when those structures break down, fluid can collect in the wrong place.
Beneath the retina sits a thin layer of cells called the retinal pigment epithelium, or RPE. The RPE acts as a barrier between the retina and the choroid, a layer of blood vessels that supplies nutrients to the outer retina. When the RPE is healthy, it keeps fluid from crossing into the space beneath the retina.
In CSR, small defects develop in the RPE barrier. Fluid from the choroid leaks through these weak points and pools beneath the retina, creating what is called a serous detachment. When this pocket of fluid forms under the macula (the central zone of the retina responsible for sharp, detailed vision), central vision becomes blurry or distorted.
The exact cause is not fully understood. Researchers believe the process begins with dysfunction in the choroid itself, leading to increased pressure or abnormal permeability in the blood vessels beneath the RPE. This abnormal choroidal activity pushes fluid through weak points in the barrier.
Corticosteroid exposure is one of the strongest known triggers. Corticosteroids are found in many common medications, including allergy nasal sprays, anti-inflammatory skin creams, oral medications for autoimmune conditions, and joint injections. Even over-the-counter products containing corticosteroids may contribute to CSR in susceptible individuals.
Most cases are classified as acute, meaning the fluid resolves within about three months and vision often returns to near-normal levels. Chronic CSR is diagnosed when fluid persists beyond three to four months or when widespread RPE changes are present. Chronic CSR carries a higher risk of lasting vision changes because prolonged fluid exposure can damage the photoreceptor cells, the specialized cells in the retina that detect light.
Who Is at Risk
CSR does not affect everyone equally. Certain groups face a noticeably higher risk, and understanding those risk factors can help with early detection and prevention.
CSR primarily affects adults between the ages of 20 and 55. Men are significantly more likely to develop the condition than women, with some estimates suggesting men are affected roughly five to six times more often. The reasons behind this difference are not yet fully explained.
Using corticosteroids in any form is a well-established risk factor. This includes oral steroids, inhaled steroids for asthma, topical steroid creams, steroid nasal sprays, and steroid injections into joints or the spine. People with conditions that cause the body to produce excess cortisol naturally, such as Cushing syndrome, also face an elevated risk.
Emotional stress has long been associated with CSR. Researchers have observed a relationship between CSR and so-called type A personality traits, which include high levels of competitiveness, urgency, and drive. During periods of stress, the body may produce higher levels of cortisol, a natural corticosteroid, which may trigger CSR in people who are predisposed to the condition.
Several additional factors may increase the likelihood of developing CSR.
- Use of phosphodiesterase-5 inhibitor medications, such as those prescribed for erectile dysfunction
- Use of certain medications for autoimmune diseases
- Diabetes mellitus
- A family history of CSR, with roughly half of diagnosed patients having at least one relative with related retinal findings
If you share several of these risk factors, routine eye examinations become especially important for early detection.
Signs and Symptoms
CSR can present in different ways depending on where the fluid collects and how much has accumulated. Some people notice dramatic changes in vision right away, while others have little or no awareness of the condition.
The most common symptom is blurry central vision, usually in one eye. Many patients also notice that straight lines appear wavy or bent, a symptom called metamorphopsia. Objects may appear smaller than usual in the affected eye, which is known as micropsia. Some patients also develop a mild shift toward farsightedness in the affected eye.
Some people with CSR notice a dim or dark area in the center of their vision, called a central scotoma. Colors may appear washed out or duller in the affected eye compared to the other. These changes can make reading, driving, and recognizing faces more difficult.
CSR does not always produce obvious symptoms. When fluid collects outside the macula, vision may seem entirely normal. A retina specialist may discover these cases during a routine eye examination. Even when vision appears unaffected, long-term fluid accumulation can still cause underlying damage over time. Careful examination also often reveals subtle involvement in the fellow eye.
Diagnosis and Testing
Diagnosing CSR requires a thorough examination of the retina combined with specialized imaging. Our retina specialists use several tests to confirm the diagnosis, locate the source of leakage, and guide treatment decisions.
A retina specialist will begin by dilating the pupils and examining the back of the eye. During this examination, a dome-shaped elevation caused by fluid beneath the retina may be visible. The specialist will also look for changes in the RPE, such as pigment clumping or small pigment epithelial detachments.
Optical coherence tomography, or OCT, is a noninvasive imaging test that produces detailed cross-sectional images of the retina. OCT clearly shows the pocket of fluid beneath the retina and can measure its size with precision. This test is essential for confirming the diagnosis, monitoring changes over time, and assessing how well treatment is working.
Fluorescein angiography involves injecting a yellow dye into a vein and photographing how it moves through the retinal and choroidal blood vessels. In CSR, this test reveals one or more focal points where dye leaks through the RPE. The classic appearance is an expanding dot of dye, sometimes described as a smokestack or inkblot pattern. This test helps pinpoint the exact source of leakage.
Indocyanine green (ICG) angiography uses a different dye that is better suited to imaging the deeper choroidal blood vessels beneath the RPE. This test can reveal areas of choroidal congestion and abnormal permeability that may not show up on fluorescein angiography. ICG angiography is particularly useful for evaluating chronic or recurring CSR and for planning certain laser-based treatments.
Treatment Options
Treatment for CSR depends on whether the condition is acute or chronic, how much vision is affected, and the location of the fluid. Our retina specialists develop individualized treatment plans based on each patient's specific situation.
Because most acute cases resolve on their own within three months, many retina specialists initially recommend careful monitoring. During this period, patients should work with their prescribing physicians to reduce or discontinue corticosteroid medications if it is medically safe to do so. It is critical not to stop high-dose steroid medications suddenly without medical supervision, as this can cause serious health problems. Stress management practices may also be beneficial during the observation period.
Photodynamic therapy (PDT) is one of the most effective treatments for chronic CSR. A light-sensitive medication called verteporfin is injected into a vein and travels through the bloodstream to the abnormal blood vessels in the choroid. A retina specialist then applies a low-energy laser to the area of leakage, which activates the medication and helps seal the abnormal leakage points without significantly damaging the overlying retina.
The preferred approach uses half the standard dose or half the standard laser energy, a protocol that reduces the risk of side effects while remaining effective at reducing abnormal choroidal activity. This is currently considered a leading treatment option for chronic CSR.
Mineralocorticoid receptor antagonists (MRAs) are a class of oral medications that have shown promise for chronic and recurring CSR. Eplerenone, originally approved for high blood pressure, has demonstrated the ability to improve visual acuity and reduce fluid accumulation in the macula in clinical studies.
MRAs may be appropriate as a first-line, noninvasive option for patients who prefer to avoid laser-based treatments or who are not good candidates for them. A retina specialist will monitor kidney function and potassium levels during treatment, as these are important safety considerations with this type of medication.
In selected cases, a retina specialist may use a focused thermal laser to seal a specific leakage point identified on fluorescein angiography. This approach is generally reserved for leakage points located well away from the center of the macula, because the laser can create a small permanent blind spot. Thermal laser is used less often today because photodynamic therapy provides similar effectiveness with a lower risk to central vision.
Anti-VEGF medications block a protein called vascular endothelial growth factor, which drives abnormal blood vessel growth and leakage. These medications are more clearly beneficial when CSR is complicated by choroidal neovascularization, meaning abnormal new blood vessel growth beneath the retina. For CSR without this complication, research on anti-VEGF therapy has shown mixed results, and these injections are not typically the first choice for straightforward cases.
What to Expect Over Time
Understanding how CSR typically progresses can help patients prepare for recovery and recognize when to seek follow-up care. The outlook varies depending on whether the condition is acute or chronic and whether it recurs.
Most people with a first episode of acute CSR experience meaningful vision improvement within one to three months as the fluid gradually absorbs on its own. Many patients recover to 20/20 or close to normal vision. However, some notice subtle lasting differences between the two eyes, including slight changes in color perception, contrast sensitivity, or mild residual distortion, even after the fluid has completely resolved.
Recurrence is common with CSR. Up to half of untreated cases may experience a return of symptoms at some point. Recurrences can happen months or years after the initial episode and may affect the same eye or the opposite eye. Patients who have had CSR should remain alert to any changes in vision and seek prompt evaluation if familiar symptoms return.
Chronic CSR carries a less favorable prognosis than acute CSR. Prolonged or repeated episodes of fluid under the retina can lead to progressive damage to the photoreceptor cells and RPE. Over time, this may result in lasting reductions in central visual acuity, contrast sensitivity, and the ability to perceive fine detail. Early treatment of chronic CSR is important to minimize the risk of irreversible changes.
Living With Central Serous Retinopathy
Ongoing self-care and awareness play an important role in managing CSR between appointments. There are practical steps patients can take to reduce their risk of recurrence and catch any changes early.
The most important step a patient with CSR can take is to avoid corticosteroid medications whenever it is medically appropriate. This applies not only to prescription steroids but also to over-the-counter nasal sprays and skin creams that contain corticosteroids. Informing all healthcare providers about a history of CSR allows the care team to consider steroid-free alternatives for common conditions like allergies and skin inflammation.
Because stress may play a role in triggering CSR, many retina specialists encourage patients to make stress management part of their daily routine. Regular physical activity, adequate sleep, mindfulness practices, and reducing work-related pressure may all be helpful. While no approach can guarantee prevention of recurrence, these habits support overall health and may help lower cortisol levels over time.
Patients who have had CSR can use an Amsler grid to monitor their vision between appointments. An Amsler grid is a simple chart printed with a pattern of straight lines and a central dot. By covering one eye at a time and focusing on the center dot, patients can check for new areas of waviness, blurriness, or distortion. Any new changes should be reported to a retina specialist promptly.
When to See a Retina Specialist
Knowing when to seek evaluation is an important part of managing CSR. Some situations call for a routine appointment, while others require urgent attention.
Anyone who experiences new blurry or distorted vision in one eye should schedule an evaluation with a retina specialist. This is especially important for individuals who have previously been diagnosed with CSR, those who use corticosteroid medications, and men between the ages of 20 and 55. Early evaluation helps distinguish CSR from other conditions that may require different treatment.
Certain symptoms should prompt immediate evaluation without delay. Seek urgent care right away if you experience any of the following.
- A sudden increase in floaters, which are spots or strands drifting across your vision
- Flashes of light in your peripheral (side) vision
- A shadow or dark curtain spreading across part of your visual field
- Sudden loss of vision in one eye
These symptoms may indicate a retinal tear, retinal detachment, or another serious condition that requires emergency treatment. While CSR itself is not typically an emergency, these warning signs must never be ignored.

If symptoms persist beyond three months, if fluid is located directly under the center of the macula, or if you have a history of previous episodes, earlier treatment may be recommended. The timing is always individualized and depends on factors like the amount and location of fluid, how long symptoms have been present, and whether there are signs of chronic changes.
Having CSR in one eye does not guarantee the other eye will be affected, but subtle changes in the fellow eye are frequently found on careful examination. In some patients, CSR develops in the opposite eye at a different point in time. This is one reason regular follow-up with a retina specialist is important even during periods when vision feels normal in both eyes.
There is currently no strong scientific evidence that any specific dietary supplement can treat or prevent CSR. Unlike age-related macular degeneration, CSR does not have established nutritional treatment guidelines. A heart-healthy, balanced diet supports overall vascular health, which may indirectly be beneficial. Supplements should not replace medical evaluation and treatment by a retina specialist, and patients should discuss any supplements they are taking with their care team.
Stopping certain corticosteroid medications abruptly can be medically dangerous, particularly if you are on high doses or have been taking them for an extended period. Sudden withdrawal can trigger serious health effects unrelated to the eye. If you believe a corticosteroid medication may be contributing to your CSR, the right step is to speak with both your retina specialist and the physician who prescribed the medication, so that a safe tapering plan or alternative treatment can be arranged together.
