What Is Fluorescein Angiography?
Fluorescein angiography (FA) is a specialized eye imaging procedure that maps blood flow through the retina and the layer beneath it called the choroid. It uses an injected dye and a specially designed camera to reveal details about blood vessels that standard eye exams and other imaging tests cannot show.
During the procedure, a small amount of fluorescein dye is injected into a vein in your arm. The dye travels through your bloodstream and reaches the blood vessels in your eyes within seconds. A retinal camera then takes a rapid series of photographs as the dye passes through.
The fluorescein dye glows brightly when exposed to blue light from the camera. Healthy blood vessels keep the dye contained within their walls. Abnormal or fragile vessels allow the dye to leak out into surrounding tissue, and this leakage appears as expanding bright areas in the images. Retina specialists review these images in early and late phases to understand how blood is moving and where problems are occurring.
Optical coherence tomography (OCT) is another imaging test commonly used in AMD care. OCT creates detailed cross-sectional pictures of the retina's layers and is very effective at detecting fluid buildup and measuring retinal thickness. However, it does not directly show whether blood vessels are actively leaking or identify the precise source of that leakage.
Fluorescein angiography and OCT provide different, complementary types of information. While OCT is often used for routine monitoring because it does not require a dye injection, fluorescein angiography remains essential for classifying AMD, especially the wet form, and for answering questions that OCT alone cannot resolve.
Why This Test Matters for AMD
Fluorescein angiography is particularly important in the care of AMD because the disease exists in two very different forms that require very different responses. Identifying which form is present, and understanding exactly how it is behaving, directly shapes every decision about your care.
Dry AMD involves the gradual breakdown of light-sensitive cells in the macula. Wet AMD occurs when abnormal new blood vessels grow beneath the retina and begin leaking fluid or blood, which can rapidly damage central vision. Fluorescein angiography is the most effective test for confirming wet AMD and distinguishing it from the dry form.
This distinction matters greatly because wet AMD requires prompt treatment to prevent serious vision loss, while dry AMD is managed differently and monitored over time.

Who Should Have This Test
Fluorescein angiography is not needed by every patient with AMD at every visit. Our specialists recommend it when it will provide information that changes or refines your care in a meaningful way.
Fluorescein angiography is recommended for patients who have AMD and develop symptoms that may indicate CNV. These include metamorphopsia (distorted or wavy vision), a new scotoma (a dark or missing spot in central vision), or unexplained vision loss. Clinical signs such as retinal bleeding, fluid beneath the retinal pigment layer, or fatty deposits from leaking vessels also prompt evaluation.
Not all patients notice symptoms immediately when wet AMD begins. Those who are at high risk for progression from dry to wet AMD may need this test even without new complaints, based on findings during a dilated eye exam.
AMD is the leading cause of significant central vision loss in people over age 50 in developed countries. Risk factors for progression include advancing age, smoking, family history of AMD, and certain genetic factors.
Patients already diagnosed with AMD in one eye are at higher risk for developing the condition in the other eye. What to Expect During the Procedure
Most patients find fluorescein angiography to be a straightforward experience. Knowing what will happen at each step can help you feel more comfortable on the day of your appointment.
Our team will review your medical history and ask about any known allergies, including prior reactions to dyes. Dilating eye drops will be placed in your eyes to widen the pupils, which allows the camera to capture the full retina. Dilation usually takes 20 to 30 minutes to reach full effect.
We recommend bringing sunglasses to your appointment, as your eyes will remain sensitive to light for several hours afterward. You may also want to arrange for someone to drive you home, since your vision will be temporarily blurred from the dilation drops.
A technician will place a small needle into a vein in your arm or hand, similar to a standard blood draw. The fluorescein dye is then injected, and within seconds it reaches the blood vessels in your eyes. The camera takes a rapid series of photographs as the dye flows through your retinal circulation.
You may be asked to look in different directions so the photographer can capture all areas of the retina. Most patients notice a brief warm sensation when the dye is injected. The imaging process itself is not painful, and the entire procedure generally takes about 10 minutes. Additional late-phase images are sometimes captured five to ten minutes after the initial injection.
After your appointment, your skin may appear slightly yellow for a few hours, and your urine will turn bright orange or yellow for one to two days. This is a normal sign that your body is clearing the fluorescein dye, and drinking plenty of water helps speed that process along.
Most patients return to their normal activities the same day. Your retina specialist will review the images and discuss the results with you, either at the same visit or at a scheduled follow-up appointment.
Fluorescein angiography is considered a safe procedure for the vast majority of patients. The most common side effect is mild, temporary nausea, which affects a small percentage of patients. Rarely, some patients may experience an allergic reaction to the dye, ranging from mild hives to more serious responses.

In these cases, FA can reveal whether leakage is still active, identify new areas of CNV that have developed, or confirm that the abnormal vessels have been successfully closed by treatment. This information directly influences the next steps in your care plan.
Fluorescein angiography is not needed at every clinic visit. For many patients with wet AMD, the test is performed at diagnosis and then repeated only when there is a specific clinical reason, such as a sudden change in vision or evidence of disease progression despite treatment.

Warning Signs That Need Prompt Attention

If you experience a sudden increase in floaters, new flashes of light, a curtain or shadow across your vision, or sudden vision loss in either eye, seek care right away. These symptoms may signal a serious change in your condition that needs same-day evaluation. Do not wait for a routine appointment if you notice these warning signs.
If you have been diagnosed with dry AMD and begin to notice that straight lines look wavy or bent (metamorphopsia), a dark or blurry area in the center of your vision, or difficulty reading even with your current glasses, contact our office promptly.
