AMD comes in two forms. Dry AMD is the more common type and develops gradually as the macula thins and drusen build up. In some people, dry AMD progresses to geographic atrophy, a condition where patches of retinal cells die off and vision loss becomes more severe.

Wet AMD is less common but more aggressive. It occurs when abnormal blood vessels grow beneath the retina and leak fluid or blood, causing rapid and significant vision loss if not treated promptly. AMD always begins in the dry form and may or may not progress to the wet form. These two types involve different disease processes and require different treatments.

In its early stages, AMD often causes no noticeable symptoms at all. The disease can be present in one or both eyes without any warning. Because damage can accumulate silently, many people are unaware they have AMD until it has already affected their vision. Regular comprehensive eye exams are the most reliable way to catch AMD before it changes your daily life.

Among all the lifestyle choices linked to AMD, cigarette smoking carries the strongest and most consistent connection to the disease. Research across many large studies agrees: smoking significantly raises your risk of developing AMD and makes the disease harder to treat once it begins.

Smokers are up to four times more likely to develop AMD than people who have never smoked. Current smokers are also more likely to develop AMD up to a decade earlier than nonsmokers. Heavy, long-term smokers face an even higher risk, and the relationship between how much you smoke and how high your risk climbs is well established in the medical literature.

Cigarette smoke contains thousands of toxic chemicals, including arsenic, formaldehyde, and ammonia. When inhaled, these chemicals enter the bloodstream and reach the delicate tissues of the eye, where they damage retinal cell structures. Smoking also increases the activity of free radicals, harmful molecules that accelerate cellular aging and reduce the body's ability to repair itself. The retina is particularly vulnerable because it has one of the highest metabolic rates of any tissue in the body.

The complement cascade is a branch of the immune system that helps fight infection. Research has strongly linked overactivation of this system to AMD. Cigarette smoke can trigger this cascade in retinal cells, causing chronic inflammation and ongoing damage. People with certain genetic variations in their complement genes are especially susceptible to smoke-induced AMD. The combination of these genetic factors and smoking is estimated to account for a significant portion of all AMD cases.

Smoking reduces the effectiveness of antioxidants throughout the body and may deplete lutein, a protective pigment found in the macula that shields it from light damage. Lower lutein levels are associated with a higher likelihood of developing AMD. Smoking also reduces the oxygen and nutrient supply to the choroid, the network of blood vessels beneath the retina, by damaging small blood vessels throughout the body.

Other Risk Factors That Affect AMD

Smoking is the most important modifiable risk factor, but it is not the only one. A range of other factors, both controllable and not, influence how likely you are to develop AMD and how quickly it may progress.

Age is the single strongest overall risk factor for AMD. The likelihood of developing the disease increases significantly with each decade after 55, and the risk rises steeply for adults in their 70s and 80s. A family history of AMD also meaningfully increases your risk. While neither your age nor your genetics can be changed, knowing your family history allows a retina specialist to recommend earlier screening and closer monitoring.

A higher body mass index is associated with increased AMD risk. Regular physical activity may help lower that risk by improving cardiovascular health, reducing inflammation, and supporting healthy blood flow to the retina. Moderate exercises such as walking, swimming, and cycling are frequently recommended by retina specialists for patients managing AMD risk factors. Physical activity also helps control body weight and blood pressure, both of which matter for eye health.

Diets rich in antioxidants and healthy fats may help slow AMD progression. Key nutrients include lutein, zeaxanthin, vitamins C and E, zinc, and omega-3 fatty acids. Foods that support retinal health include the following.

  • Dark leafy greens such as spinach, kale, and collard greens for lutein and zeaxanthin
  • Colorful fruits and vegetables such as oranges, bell peppers, and berries for vitamins C and E
  • Fatty fish such as salmon, mackerel, and sardines for omega-3 fatty acids
  • Nuts, seeds, and whole grains for zinc and other micronutrients

The Age-Related Eye Disease Studies found that specific supplement formulations can reduce the risk of progression from intermediate to advanced AMD. Talk with a retina specialist before starting any supplement regimen, because not all formulations are appropriate for every patient.

High blood pressure is a recognized risk factor for AMD. The blood vessels supplying the retina are sensitive to changes in pressure, and uncontrolled hypertension can damage them over time. Working with your primary care provider to keep blood pressure, cholesterol, and blood sugar within healthy ranges also supports the long-term health of your retina.

Research shows that nonsmokers who are regularly exposed to secondhand smoke also face an elevated risk for AMD. The toxic chemicals in cigarette smoke do not only affect the person smoking. Family members who share a living space with a smoker may face a higher risk from chronic exposure. Reducing or eliminating indoor smoking benefits everyone in the household.

How Smoking Affects AMD Treatment

Smoking does not just raise the risk of developing AMD. It also reduces how well the eye responds to treatment once AMD is present. This makes quitting an important step even for patients who have already been diagnosed.

Research has found that smokers face a significantly higher risk of a poor response to intravitreal therapies, which are injections of medication directly into the eye. If you continue smoking after an AMD diagnosis, the disease is likely to progress faster and respond less well to the treatments available. Quitting smoking at any stage of AMD can improve your treatment outcomes over time.

For wet AMD, the first-line treatment involves anti-VEGF medications. These drugs block vascular endothelial growth factor, a protein that drives the growth of abnormal blood vessels beneath the retina. They are delivered by intravitreal injection and do not treat dry AMD. Commonly used anti-VEGF medications include the following.

  • Eylea (aflibercept), typically given every 4 to 8 weeks after an initial loading phase
  • Lucentis (ranibizumab), given approximately every 4 weeks
  • Vabysmo (faricimab), a bispecific antibody targeting both VEGF and Ang-2, given every 4 to 16 weeks
  • Avastin (bevacizumab), FDA-approved for cancer and used off-label for eye conditions, given every 4 to 6 weeks
  • Eylea HD (high-dose aflibercept), allowing extended dosing intervals of 8 to 16 weeks after the loading phase

Anti-VEGF treatment can improve vision in roughly one in three people with wet AMD and stabilize vision in about nine in ten. However, a meaningful number of patients do not achieve an optimal response, which underscores how important prevention and lifestyle changes are.

Geographic atrophy is the advanced form of dry AMD. Two complement inhibitors are now FDA-approved specifically to slow its progression. Syfovre (pegcetacoplan) targets the C3 complement pathway and is given monthly or every other month. Izervay (avacincaptad pegol) targets the C5 complement pathway and is given monthly. These treatments slow the rate of retinal cell loss but do not restore vision that has already been lost. They are not used for wet AMD, and anti-VEGF drugs are not used for geographic atrophy. For earlier stages of dry AMD without geographic atrophy, no approved drug currently reverses the condition, making lifestyle modification especially important.

Quitting smoking is the single most impactful lifestyle change you can make to reduce your AMD risk. Benefits begin as soon as you stop. Although it may take years for your risk to approach that of a lifelong nonsmoker, every smoke-free day reduces ongoing damage to your retina. Talk with your primary care provider about smoking cessation programs, nicotine replacement therapy, or prescription medications that can help you quit.

Focus on a diet that delivers the nutrients your macula needs to stay healthy. Prioritize dark leafy greens, colorful vegetables and fruits, fatty fish, and whole grains. If you have intermediate AMD, ask a retina specialist whether an AREDS2 supplement formulation is appropriate for you, as not every patient with AMD is a candidate for supplementation.

Aim for at least 150 minutes of moderate-intensity exercise per week, such as brisk walking, cycling, or swimming. Regular activity improves cardiovascular health, which in turn supports healthy blood flow to the retina. Physical activity also helps manage weight and blood pressure, both of which are recognized AMD risk factors.

Work with your doctor to keep blood pressure, cholesterol, and blood sugar within recommended ranges. These conditions directly affect the blood vessels that supply your retina. If you have had cataract surgery, make sure your retina specialist is aware of your full eye history, as this information helps guide your ongoing care.

Because early AMD often causes no symptoms, regular comprehensive eye exams are essential for detection before vision loss occurs. If you are over 50, have a family history of AMD, or have other risk factors such as a history of smoking, a retina specialist can use imaging tests to detect subtle changes in the macula well before you notice a problem.

When to See a Retina Specialist

Knowing when to schedule a routine visit and when to seek urgent care can make a significant difference in your outcomes. AMD can change, and timely evaluation is always the right call when something feels different with your vision.

If you have been diagnosed with early or intermediate AMD, regular visits with a retina specialist are important to track any changes. Our retina specialists, Dr. Gaurav Gupta and Dr. Pranjal Thakuria, use optical coherence tomography (OCT), a noninvasive imaging test that creates detailed cross-sections of the retina, along with dilated eye exams, to monitor the macula over time and catch any progression early.

Certain symptoms require urgent evaluation and should not be waited out. Seek care right away if you notice any of the following.

  • A sudden increase in floaters, which are spots, strings, or shadows drifting across your vision
  • Flashes of light in one or both eyes
  • A curtain, shadow, or dark area covering part of your vision
  • Sudden vision loss in one eye
  • Straight lines appearing wavy or distorted, which is known as metamorphopsia

These symptoms may indicate a transition from dry to wet AMD or another serious retinal condition that requires prompt treatment.

If you currently smoke and have been diagnosed with any stage of AMD, discuss smoking cessation with both your retina specialist and your primary care provider. Quitting can slow disease progression and improve how your eyes respond to treatment. Acting before symptoms worsen gives you the best chance of preserving the vision you have.

Your risk begins to decline soon after you stop smoking, but it does not immediately drop to the level of someone who has never smoked. There is not yet enough long-term research to confirm that e-cigarettes carry the same level of AMD risk as traditional cigarettes, but they are not considered safe for eye health. E-cigarettes still deliver nicotine and other compounds that affect blood vessel function and oxidative stress in the body. A retina specialist would not recommend vaping as a safe alternative to smoking when it comes to protecting your retinal health. The safest choice for your eyes and your overall health is to avoid nicotine products entirely.

Having AMD in one eye does place you at higher risk of developing it in the other. The timing and severity can vary significantly between eyes, which is why regular monitoring of both eyes is important. Your retina specialist may recommend more frequent imaging visits and may discuss preventive measures, including AREDS2 supplements if appropriate, to help slow or delay progression in the less affected eye.

Yes. An Amsler grid, a simple grid pattern available from your eye care provider or online, can help you notice changes in your central vision at home. Cover one eye and look at the center dot. If any lines appear wavy, blurry, or missing, this is a sign that something may have changed and you should contact your eye care provider promptly. This is especially useful for patients already diagnosed with AMD, as it can help detect a transition toward wet AMD earlier than waiting for your next scheduled visit.

If you smoke, quitting is by far the most impactful action you can take. Smoking is the largest modifiable risk factor for AMD, it accelerates progression, and it reduces how well treatments work. If you do not smoke, maintaining a diet rich in leafy greens and omega-3 fatty acids, staying physically active, managing blood pressure, and keeping up with regular eye exams are your most effective tools.
