How Sarcoidosis Affects the Eyes
Sarcoidosis causes the immune system to form tiny clusters of inflamed cells called granulomas in various organs throughout the body. The eyes are among the most commonly affected organs, and eye symptoms can appear before, during, or after other signs of the disease show up elsewhere.
Sarcoidosis is a systemic inflammatory condition in which the immune system produces small clumps of inflamed tissue called granulomas. These granulomas can form in the lungs, lymph nodes, skin, liver, and eyes. Ocular involvement occurs in a significant percentage of people with systemic sarcoidosis, making regular eye evaluations an essential part of managing the condition.
Sarcoidosis can occur in children and adolescents, though it is more common in adults. Women with sarcoidosis tend to develop eye symptoms more frequently than men.
Uveitis, which is inflammation inside the eye, is the most frequent eye complication of sarcoidosis. Anterior uveitis, meaning inflammation in the front chamber of the eye, is the most common type seen in sarcoidosis patients. In many cases, uveitis is actually the first sign that sarcoidosis is present, appearing before any other symptoms of the disease develop.
This means a Pediatric Ophthalmologist may be the first specialist to suspect sarcoidosis based on the pattern of inflammation seen during an eye exam. Early identification gives patients the best chance of protecting their vision and managing the disease before it progresses.
Sarcoidosis does not always stop at uveitis. It can also affect other structures in and around the eye, including:
- Conjunctival nodules, which are small raised bumps on the white of the eye
- Lacrimal gland enlargement, which can cause dry eye symptoms
- Optic nerve involvement, including swelling or granuloma formation on the nerve
- Orbital inflammation that can cause the eye to protrude slightly
- Epiretinal membranes, which are thin layers of tissue that form over the central retina and can affect vision

Diagnosing Ocular Sarcoidosis
Diagnosing sarcoidosis as the cause of eye inflammation requires a careful combination of clinical examination, laboratory testing, and sometimes imaging or tissue biopsy. No single test confirms the diagnosis on its own, so the full picture matters.
During a slit-lamp examination, your doctor looks for specific patterns of inflammation that are characteristic of sarcoidosis. These include the size, shape, and location of inflammatory deposits inside the eye, as well as signs of damage to surrounding structures. The pattern of uveitis seen in sarcoidosis is often distinct enough to guide the diagnostic workup before any lab results return.
When sarcoidosis is suspected based on the eye exam, a systemic workup is typically recommended. This usually includes:
- Chest X-ray or CT scan to look for enlarged lymph nodes or lung involvement
- Serum ACE (angiotensin-converting enzyme) level, a blood test that is often elevated in active sarcoidosis
- Lysozyme measurement, another blood marker associated with granulomatous disease
- Tuberculin skin testing or interferon-gamma release assay to rule out tuberculosis
- Syphilis screening, since syphilis can cause similar patterns of eye inflammation
Bilateral hilar lymphadenopathy, which means enlarged lymph nodes on both sides of the chest visible on imaging, is a strong indicator of sarcoidosis even without a tissue biopsy.
Definitive diagnosis of sarcoidosis requires evidence of noncaseating granulomas (a specific type of inflammatory cluster without a certain pattern of tissue breakdown) on biopsy. Accessible sites such as the conjunctiva, eyelid, or lacrimal gland can provide tissue for evaluation when needed. Not every patient requires a biopsy, particularly when the clinical and imaging findings are strongly consistent with sarcoidosis and other causes have been ruled out.
Sarcoid uveitis can be present and causing damage even when it produces no noticeable symptoms in its early stages. Anyone newly diagnosed with systemic sarcoidosis should have a complete eye evaluation, even if their eyes feel fine. Untreated sarcoid uveitis can lead to serious complications including glaucoma, cataracts, and permanent vision loss.
Early referral to a specialist experienced in inflammatory eye disease improves long-term outcomes significantly. Factors associated with a more challenging prognosis include delayed referral, inflammation affecting the middle or back of the eye, fluid accumulation in the central retina, and secondary glaucoma.
Treatment Options for Sarcoid Eye Disease
Treatment for ocular sarcoidosis is tailored to the type and severity of inflammation. The goal is to reduce inflammation, prevent complications, and protect long-term vision while minimizing the side effects of treatment.
For anterior uveitis, meaning inflammation limited to the front of the eye, topical steroid eye drops are the first-line treatment. Prednisolone acetate 1% drops are commonly prescribed, starting at a higher frequency and gradually tapered as the inflammation resolves. Most patients with isolated anterior disease respond well to topical therapy alone.
Intraocular pressure must be monitored closely during topical steroid treatment, since steroids can sometimes cause pressure to rise and lead to steroid-induced glaucoma. Your doctor checks eye pressure at every follow-up visit and adjusts treatment if needed.
When inflammation involves the middle or back of the eye, affects both eyes, or does not respond adequately to drops, systemic treatment is almost always necessary. Oral prednisone is typically used for initial control and then tapered as steroid-sparing medications are introduced.
Steroid-sparing medications help reduce inflammation without the long-term risks of high-dose steroids. Commonly used options include:
- Methotrexate, which is the most widely used agent for chronic sarcoid uveitis
- Mycophenolate mofetil
- Azathioprine
- Cyclosporine
Your doctor selects the most appropriate option based on your response to initial treatment, how well the medication is tolerated, and whether other organs are also involved.
When sarcoid uveitis does not respond adequately to conventional immunosuppressive medications, biologic therapy may be recommended. Adalimumab is the only FDA-approved biologic specifically for noninfectious uveitis, and it has shown effectiveness in sarcoid-related eye inflammation that has been difficult to control with other treatments. Infliximab is another option used in refractory cases.
Biologic therapy is typically managed in coordination between your ophthalmologist and rheumatologist, with close monitoring throughout treatment.
Long-Term Management and Follow-Up
Sarcoid uveitis often follows a chronic course that requires ongoing monitoring and care over many years. Staying connected with your care team is one of the most important things you can do to protect your vision.
The frequency of follow-up visits depends on disease activity. During active inflammation, monthly visits are often needed. When the disease is well controlled, visits every three to six months may be appropriate. Your doctor uses slit-lamp examinations to detect flare-ups before they cause additional damage and to check for complications at every visit.
Complications that can develop from the disease itself or as side effects of treatment include:
- Glaucoma, which is elevated eye pressure that can damage the optic nerve
- Cataracts, which is clouding of the natural lens inside the eye
- Cystoid macular edema, which is swelling in the central retina that can blur central vision
Sarcoidosis can affect the lungs, skin, lymph nodes, and other organs at the same time it is affecting the eyes. Your child's care team may include a Pediatric Ophthalmologist, a rheumatologist, and a pulmonologist working together. Each specialist needs to know what medications the others are prescribing, since changes in systemic treatment can affect eye management and vice versa.

Many patients with sarcoid uveitis respond well to treatment when it begins early and is maintained consistently. The outlook is generally more favorable for isolated anterior uveitis than for inflammation involving the middle or back of the eye. Some patients achieve remission and are able to taper off medications gradually under close supervision. Others require long-term maintenance therapy to prevent relapse and preserve their vision.
Consistent follow-up and adherence to the treatment plan are among the strongest predictors of good visual outcomes over the long term.
