How AMD Is Staged
AMD staging gives your eye doctor a clear picture of how far the condition has progressed and what level of care is appropriate right now. The stage you are assigned today shapes your monitoring schedule, supplement decisions, and treatment plan.
Eye doctors stage AMD using a framework developed through the Age-Related Eye Disease Studies, commonly called AREDS and AREDS2. This system evaluates the size and number of drusen, which are small yellow deposits that form beneath the retina, as well as any changes in pigmentation in the macula. These findings together determine which stage of AMD you have.
After dilating your pupils, your doctor examines your retina closely. Drusen size is the primary factor in staging. Small drusen under 63 micrometers are considered part of normal aging. Medium drusen measure 63 to 124 micrometers. Large drusen reach 125 micrometers or more and signal a higher risk of progression.
Pigment changes in the macula also affect your stage. Dark clumps of pigment (hyperpigmentation) or pale areas where pigment has faded (hypopigmentation) can raise your risk of advancing to a later stage, even when drusen size alone might suggest otherwise.
Your AMD stage is not just a label. It tells your doctor how to monitor you, whether AREDS2 supplements are appropriate, and whether active treatment is needed. Early AMD calls for monitoring only. Early AMD
Early AMD is the first recognizable stage of the condition, detected through imaging and dilated examination before any vision changes occur. Understanding what this stage means helps you stay engaged in your own monitoring and know what to watch for over time.
Early AMD is characterized by medium-sized drusen with no pigment changes and no areas of retinal cell loss. Your retina is changing at a cellular level, but your central vision remains clear. Most people at this stage feel nothing unusual with their eyes and are often surprised when their doctor identifies the condition during a routine exam.
Early AMD typically produces no noticeable symptoms. The condition can only be identified through a dilated eye exam or imaging, such as optical coherence tomography (OCT), which captures detailed cross-sections of the retina. Your doctor will likely recommend exams every 12 to 24 months to monitor for any changes.
No active treatment is needed at this stage. AREDS2 supplements are not recommended for early AMD because clinical research did not show a benefit for people with only small or medium drusen and no pigment changes.
Most people with early AMD do not progress to late AMD quickly. Your individual risk depends on several factors, including how many drusen are present, whether both eyes are affected, and whether you have risk factors such as smoking or a family history of AMD. Regular exams allow your doctor to catch any shift toward intermediate AMD early, when the care plan can be adjusted promptly.
Intermediate AMD
Intermediate AMD is the stage when the risk of vision-affecting progression becomes meaningful. This is also the stage where preventive action, in the form of AREDS2 supplements, has been shown to help. Recognizing this stage and understanding your options makes a real difference in outcomes.
Intermediate AMD is defined by the presence of large drusen measuring 125 micrometers or more, by pigmentary abnormalities in the macula, or by a combination of both. Some patients at this stage also have early geographic atrophy, a form of retinal cell loss that has not yet reached the fovea, the very center of the macula where detailed vision originates. Your doctor may identify these findings through dilated examination, OCT imaging, or fundus photography.
Some people with intermediate AMD start to notice mild blurring, reduced contrast, or difficulty adjusting to low light conditions. These changes can develop so gradually that they are easy to overlook or attribute to normal aging.
If reading requires noticeably more light than it used to, or if colors seem less vivid than before, share these observations with your doctor at your next visit. Correlating your symptoms with imaging findings helps your care team track how quickly the condition is progressing.
For people with intermediate AMD, AREDS2 supplements have been shown to reduce the risk of progressing to late AMD by approximately 25 percent over five years. The formula contains vitamins C and E, zinc, copper, lutein, and zeaxanthin. These nutrients support the health of retinal cells under stress from AMD.
Your doctor determines whether AREDS2 is appropriate for you based on your drusen burden, pigment changes, and overall health history. Because zinc in the formula can interact with certain medications, always inform your doctor about any other supplements or prescriptions you are taking before starting AREDS2.
Late AMD
Late AMD is the stage at which vision loss becomes more significant and treatment becomes necessary. There are two distinct forms of late AMD, and each has a different presentation, pace, and treatment approach. Knowing the difference helps you understand your diagnosis and respond quickly when needed.
Geographic atrophy (GA) occurs when patches of retinal cells in the macula gradually die, leaving well-defined areas of thinning. These patches expand slowly over months to years. When atrophy reaches the fovea, central vision can be significantly affected.
Treatments are available that have been shown to slow the growth of geographic atrophy lesions. These therapies do not restore vision that has already been lost, but slowing lesion growth can help protect remaining function. Your doctor weighs the potential benefit against the commitment of ongoing treatment to determine the right approach for you.
Wet AMD occurs when abnormal blood vessels grow beneath the retina and leak fluid or blood. This process, called choroidal neovascularization, can cause sudden central blurring, straight lines that appear wavy, or dark spots in the center of your vision. Without prompt treatment, wet AMD can cause severe vision loss within a short period of time.
Anti-VEGF injections, which block the growth signals that drive abnormal vessel development, are the primary treatment for wet AMD. When started early and maintained on schedule, these injections stabilize or improve vision in the majority of patients. Consistent follow-through with your treatment schedule is one of the most important things you can do to protect your sight.

AMD stages do not reverse. Drusen may occasionally appear smaller on imaging, but the underlying retinal changes that define your stage remain. Treatment aims to slow or stop further progression, not to undo what has already occurred. If your imaging findings shift, your doctor may reclassify your stage to reflect current findings, but this is a change in measurement, not a true reversal of the disease.
The pace of AMD progression varies considerably from person to person. Some individuals remain at the early stage for a decade or more without significant change. Others advance to intermediate or late AMD within a few years. Your drusen burden, the presence of pigment changes, your genetics, smoking history, and cardiovascular health all influence how quickly AMD may progress. This variability is exactly why consistent monitoring matters so much.
AREDS2 supplements are available without a prescription, but that does not mean they are appropriate for everyone with AMD. The formula was studied specifically in people with intermediate AMD or late AMD in one eye. Taking it at an earlier stage has not been shown to provide benefit. The zinc component can also interact with certain antibiotics and other medications. Wet AMD typically progresses much faster than dry AMD and can cause significant vision loss within weeks if untreated, which makes it a more urgent clinical situation.
