How Eylea Works
Eylea targets the underlying process that causes vision loss in several serious retinal diseases. It works by interrupting the chain of events that leads to fluid buildup and swelling in the back of the eye.
Many retinal conditions involve a protein called vascular endothelial growth factor, or VEGF. When the retina is damaged or under stress, it produces excess VEGF, which signals the body to grow new blood vessels in the eye. These abnormal blood vessels are fragile and tend to leak fluid or blood into the retina, causing swelling in the macula, the part of the retina responsible for sharp central vision. Without treatment, this swelling can damage retinal cells and lead to lasting vision loss.
Aflibercept works as a decoy receptor, sometimes called a VEGF trap. It binds to VEGF-A and placental growth factor (PlGF) before these proteins can reach blood vessel cells and trigger abnormal growth. By capturing these proteins, aflibercept stops the signal that drives leakage and swelling in the retina. Over time, the retina can begin to dry out and stabilize, and many patients experience improved or stabilized vision as a result.
The FDA has approved Eylea for several retinal conditions, all of which involve abnormal VEGF activity. These include wet age-related macular degeneration (wet AMD), diabetic macular edema (DME), macular edema following retinal vein occlusion (RVO), diabetic retinopathy (DR), and myopic choroidal neovascularization (myopic CNV), which is abnormal blood vessel growth related to severe nearsightedness. What the Clinical Evidence Shows

What the Injection Experience Is Like

Your retina specialist will numb your eye using anesthetic drops or a small injection of numbing medication before any other steps are taken. Eylea HD (high-dose aflibercept, 8 mg) was approved by the FDA in 2023 for wet AMD, DME, and diabetic retinopathy, and received additional FDA approval for retinal vein occlusion in November 2025. This higher-dose formulation allows for extended dosing intervals of 8 to 16 weeks after the initial loading phase. For many patients, this means fewer office visits and injections while maintaining the same level of vision protection.

These include eye infection (endophthalmitis), retinal detachment, increased eye pressure, and bleeding inside the eye. There is also a small risk of blood clots in rare cases.
